Patient Forms
If you are a new patient to our office, we will be mailing you our New Patient Forms Bundle with forms that will need to be filled out when you arrive at our office. Filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. Dental Insurance
We are happy to electronically submit your insurance claims to maximize your benefits. Dental plans are designed to share in the cost of your dental care, not to completely pay for those costs. We will estimate your insurance coverage. To help us with your insurance filing, please bring your dental card and information booklet with you. Remember that insurance is a benefit for you and is a contract between the subscriber and the insurance company, to which we are a third party.

Dental Financing
